As the parent/legal guardian of the above-named minor, I acknowledge that participation in physical training involves inherent risks, including injury. I voluntarily assume all risks and waive any claims against Off Grid Performance, its staff, and facility, including those arising from negligence.

I confirm the participant is in good health and authorize Off Grid Performance staff to seek emergency medical care if needed. I agree to indemnify and hold harmless Off Grid Performance from any claims or legal actions resulting from participation.

I grant permission to Off Grid Performance to take photographs and video recordings of my child during training sessions and events. These materials may be used for promotional purposes on social media, websites, and printed materials. I understand that no names or identifying information will be disclosed without additional consent.

I understand that I may revoke this consent at any time by providing written notice to Off Grid Performance.
